1899 · Emil Kraepelin
Psychiatrie: Ein Lehrbuch für Studirende und Aerzte, 6., vollständig umgearb. Aufl.
What this work is
The 1899 sixth edition of Kraepelin's psychiatry textbook. It sorts asylum patients into disease groups by their symptoms and the course of their illness, most famously dementia praecox and manic-depressive insanity. It also reports timing experiments in which his clinic measured how fast patients answered word prompts.
Then and now
Kraepelin sorted asylum patients into disease groups by symptoms and course. Psychiatry made his groups the backbone of its diagnostic manuals and treats each as a real disease. His book sorted behavior and never showed such a disease, and nobody has seen one.
Verdict
UNSCIENTIFIC
Neither the paradigm nor the work inside it passes. The verdict is unscientific.
1·This work · inside the paradigm
UNSCIENTIFIC
Does not follow the scientific method within Psychiatric Model.
2·Paradigm · Psychiatric Model
UNSCIENTIFIC
Fails the tests. The worse layer decides.
How the verdict is decided
The verdict grades both layers: the paradigm this work assumes, and how the work was carried out inside it. Both must be scientific for the whole thing to be scientific. A clean method inside an unscientific paradigm is unscientific. An unscientific method inside a better paradigm is unscientific. A split on either layer is mixed. The worse layer decides.
1·This work
Did this work test its claim with methods that are independent, falsifiable, and non-circular?
UNSCIENTIFIC
This work claims that mental illnesses fall into distinct disease groups, like dementia praecox and manic-depressive insanity, that a doctor can reliably recognize from symptoms and course. All three tests fail.
Independently Verifiable
Fail
A stranger can watch the restless patient and read the stopwatch, but dementia praecox appears only when the book's own criteria interpret what was watched.
Falsifiable
Fail
When another researcher found manic patients answering faster, Kraepelin ruled her timing faulty, so a contradicting result arrived already explained away as error.
Non-Circular
Fail
The book's criteria decide who counts as manic, and the behavior of the people so sorted is then offered as proof that mania is one disease.
Why
The stopwatch and the pacing patient are real, but dementia praecox appears only when the book's criteria read them. When Walitzkaja's timings of manic patients went against his account, he called her measurements faulty. The criteria decide who counts as manic, and the people so sorted are then the proof that mania is one disease.
2·Paradigm · Psychiatric Model
Does the framework this work assumes pass the three tests?
UNSCIENTIFIC
Independently Verifiable
Fail
Patients pacing and talking are visible; the brain disease the model reads into them appears only after the diagnostic manual speaks.
Falsifiable
Fail
Six editions redefined the disease groups and the model survived every redefinition, so no patient outcome could end it.
Non-Circular
Fail
The manual defines the diseases, doctors diagnose by the manual, and agreement among manual-trained doctors is offered as proof the diseases exist.
Why
The model takes behavior anyone can watch, a patient pacing, refusing to answer, talking without stopping, and adds the criteria, cutoffs, and names that turn it into a disease. The behavior exists without the model. Dementia praecox does not. Kraepelin rewrote his classification across six editions while declaring its foundations unshaken; each rewrite changed the criteria, never the model's survival. When another researcher's timing data contradicted his account of mania, he called the data faulty, not the account. Validation in this framework is doctors agreeing, and the doctors were trained on the same manual. The book treats its diagnosis as a thing found inside the patient, and never asks what happened to the person that produced the behavior.
From the work
